How Oral Appliance Therapy Works
A custom oral appliance gently repositions your lower jaw forward during sleep, keeping the soft tissues of your throat from collapsing — the root cause of obstructive sleep apnea and snoring.
No mask, no tubes, no forced air, no machine. Pocket-sized and silent — works anywhere in the world.

Dr. Barzilay treats the full spectrum of sleep-disordered breathing using evidence-based oral appliance therapy.
Mild to moderate OSA — and as an alternative for patients with severe OSA who cannot tolerate CPAP. Objective follow-up testing at every case.
If you have tried CPAP and cannot tolerate the mask, noise, or forced air, oral appliance therapy may provide an effective, comfortable alternative.
Even without a formal OSA diagnosis, disruptive snoring responds well to a custom oral appliance and can significantly improve sleep quality for you and your partner.
UARS patients — characterized by sleep-disrupting airway resistance without full apneas — often benefit significantly from mandibular advancement therapy.
Why Credentials Matter
Not all dentists who provide oral appliances have specialized training. Dr. Barzilay holds the gold-standard D.ABDSM designation — earned through documented clinical experience, formal examination, and ongoing recertification.
Requires documented case experience, clinical examination, and 25 hrs CE every 2 years. Fewer than 20 holders in all of Canada.
Active member. Follows AASM clinical practice parameters and AADSM scope-of-practice guidelines at every case.
Objective follow-up sleep testing conducted after every case to verify AHI reduction — not based on symptoms alone.
Over 15 years working in partnership with sleep physicians and respirologists in Hamilton and the surrounding region.
The Treatment Journey
Every patient follows a structured, AASM-compliant protocol — from diagnosis through long-term monitoring.
OAT begins with a physician-ordered sleep study. A formal OSA diagnosis is required before any appliance is fitted.
Dr. Barzilay examines your dentition, jaw anatomy, and TMJ to confirm you are a suitable candidate.
Digital or physical impressions taken. Your device is custom-fabricated and carefully fitted.
Progressive adjustment to find the position that maximally opens your airway while remaining comfortable.
Follow-up sleep study confirms your AHI has been reduced — data-verified, not symptom-based.
Ongoing follow-up for bite changes, appliance wear, and continued efficacy with regular physician communication.
